7 practical ways to reduce no-shows at a small clinic
Most no-shows are forgetfulness, not bad intent. These seven changes cost little and work within a week.
Most missed appointments are not people deciding to skip. They forgot, the time stopped working for them, or rescheduling felt like more effort than not turning up. Each fix below targets one of those reasons.
1. Send two reminders, not one
One reminder 24 hours before catches people who forgot. A second, 2 hours before, catches people whose day changed. Keep both short and include the time, the address and a one-tap way to reschedule.
2. Make rescheduling easier than not showing up
If the only way to move an appointment is a phone call during opening hours, many people will simply not come. A reschedule link in every reminder turns a no-show into a moved booking.
3. Confirm at the moment of booking
Send a confirmation immediately after booking with the date, time, practitioner and address. People who receive a confirmation are more likely to add it to their own calendar.
4. Ask for a reply
"Reply 1 to confirm, 2 to reschedule" gives you a list of people who have not answered. Call those first.
5. Keep a short waiting list
When someone cancels the day before, a waiting list of three or four patients who want an earlier slot fills the gap.
6. Use deposits carefully
Deposits reduce no-shows for long or expensive appointments, but they also put off new patients. Use them for first appointments over 45 minutes or for patients who have missed before.
7. Track it weekly
Count no-shows every week, by day and by appointment type. Patterns appear quickly. Monday mornings and long first visits are common trouble spots.
Our Clinic OS Notion template includes a no-show tracker, and The Booking Playbook has full reminder scripts.
